How to get replacement Academic Certificates in PNG

You may have  misplaced or lost or burnt your Grade 8 , 10 and 12 academic certificates and you may want to apply for a replacement certifice. PNG Education News outlines the steps to get a replacement Certificate(Statement of Result) below. Please note that you will make a declaration using a Statutory Declaration Form for your lost certificate. 

How to get Gr.8 Replacement Certificate
Grade 8 certificates are issued by Provincial Education Board. Even though the certificates are prepared by Measurement Services Division (MSD) in Port Moresby, it is a provincial function to print and issue certificates. That means, if you lost your Grade 8 certificate, do not go to MSD but go to your provincial education office and see the Grade 8 Provincial Examination Supervisor and get a copy. 

How to get Grade 10 & 12 Replacement Certificates
If you want to get Grade 10 or 12 replacement  certificates (Statement of result), download the
application form here or the link provided at the end of this post. 
You must take note of the following. 
1. Write your full name that you used at school (if changed indicate)
2. State the Grade and the year you completed
3. Name three classmates that you graduated that time.
4. Your address, Phone Numbers,
5. If you employed, state your employer
6. Enter your signature. 

The next section of the application 

7. A senior Education officer, principal , Magistrate or pastor must certify that you are geniune and completed you education on the said school. 
8. Declare your lost certificate by Completing the Statutory Declaration Form at a court house (the form is attached together with the application form below) 
9. Pay K25 application fee ( Port Moresby Residence can pay the amount at Finance Department Ground floor Vulupindi House or 4th floor Education Finance section at Fincorp Building). For other centers pay the amount at your provincial treasury office stating it is for "Statement of Result fees" for lost certificates.  Attach the receipt together with your Application form (make sure to keep a copy for yourself) and send it to
